- 香港註冊慈善機構「社群心慈善有限公司」（Community Spirit Charity Limited）成立於2013年（香港稅局檔案編號：91/13133），以關注香港及內地弱勢社群為己任，是非宗教、非政治、非牟利的慈善機構。成立以來，多次舉辦包括內地及香港的探訪及籌款活動，冀望每個人都可發揮互助互愛精神，讓社會充滿和諧、溫暖和關懷。 香港作為國際金融中心，市民普遍能安居樂業，但有一群居住在偏遠地區的長者，年事已高，部分更體弱多病，兒女都往外發展，未能經常陪伴於身邊，這群長者有些是隱蔽、殘障、長期病患、獨居或兩老互靠等，有部分僅靠微薄的生果金過活，有部分雖然領取綜援，但由於住處偏僻，醫護設施缺乏，每次出外看醫生交通費連同醫藥診金，都所費不菲，為了病痛得到醫治，唯有節衣省食，造成營養不良；加上長時間得不到適當照顧，家居潛在的危險更是令人憂慮。 一群來自不同階層的熱心市民，留意到本地獨居長者問題的嚴重性，遂聯手組識「社群心」，首項任務為香港偏遠地區之獨居長者，派發長者日常基本需要之「福袋」，派發包括食米、粟米油、通粉、麥皮、罐頭等，讓各長者除了獲得日常所需外，亦能感受義工的一份親切關懷。
