本會宗旨 本協會為一群熱心社區的專業醫療人員所創，透過『社、醫、護』三方協作，為居民提供全面醫療護理服務及運用社會工作去建構社區的溝通橋樑，以提升及增強市民預防疾病時意識及保健能力。 加強地區醫療支援服務，完善現時出入院接軌機制，使服務能更貼合居民需要。 設置危機小組及服務熱線，令居民能夠得到即時支援或跟進服務。 聯繫附近地區不同服務持份者的力量，共同為居民提供更全面的服 務。
